Zorgaanbieders en sociaal domein vormen één integraal zorgnetwerk voor en met de patiënt. Om mensen te helpen met passende zorg, op het juiste moment, dichtbij huis als dat kan en in het ziekenhuis als dat nodig is. Met aandacht voor een gezonde leefstijl en het voorkomen van ziekte. Dit is waar de NVZ en haar leden aan werken in het programma Zorg in de regio.
